FROM CONTRA COSTA COUNTY: Take Hwy 680-N across Benicia Bridge. From left lanes on bridge, take Hwy 780-W toward Benicia/Vallejo. Take Hwy 80-E toward Sacramento. Take Columbus Parkway exit to Hwy 37-W toward Napa. Take Hwy 29-N exit toward Napa. After 10 miles, turn left onto Hwy 121-W toward Sonoma. After 5 miles, turn right onto Napa Road toward Sonoma. After 4 miles, in Sonoma, turn right onto Broadway St./Hwy 12-W. Follow 12-W through the town of Sonoma, following signs to Santa Rosa. After 10 miles, pass through the town of Kenwood and past the stoplight at Pythian Avenue. At the next stoplight after Pythian, turn left onto Oakmont Avenue. FROM ALAMEDA COUNTY: Take Hwy 80-E toward Sacramento. Take Hwy 580-W/San Rafael exit. Proceed across San Rafael Bridge to San Rafael and onto Hwy 101-N toward Santa Rosa. After 37 miles, take the Hwy 12-E/Sonoma exit in Santa Rosa. When the freeway ends, turn left onto Farmers Lane (it's still Hwy 12). After 10 blocks, turn right onto Sonoma Hwy/Hwy 12. After 8 miles turn right onto Oakmont Avenue. After 4 blocks, at Laurel Leaf Place, make a U-turn and enter the 1st parking lot on the right. WeCare is in Suite C at the front of the building.
